Innovaccer Inc., a prominent healthcare artificial intelligence (AI) company, announced on September 16, 2025, its acquisition of Story Health, a pioneering AI-powered, tech-enabled platform. This strategic move is set to transform specialty care from episodic to continuous, digital models, marking Innovaccer’s fourth significant acquisition in the past two years, following Humbi AI, Cured, and Pharmacy Quality Solutions.
Story Health, founded in 2020 by Google veterans Tom Stanis, Nikhil Roy, and Cardiologist Ashul Govil, MD, MBA, developed a vertically integrated cardiovascular care platform. This platform combines AI-driven clinical pathways, advanced medication workflows, and human-led coaching to provide continuous management for conditions like heart failure, hypertension, and Atrial Fibrillation (AFib). The company had previously secured $22.6 million in Series A funding in 2022.
The core of this acquisition is Innovaccer’s vision to pioneer ‘agentic care augmentation.’ This model involves AI agents assisting specialty care teams with routine, non-clinical tasks and maintaining continuous patient engagement between clinic visits. Health coaches will work alongside these AI agents, engaging directly with patients on symptoms, medication adherence, and monitoring data from remote devices, triggering timely interventions when necessary. This integrated approach aims to embed the model within existing electronic health record (EHR) systems and workflows, enabling health systems to significantly scale specialty care capacity, reduce hospitalizations, and improve patient adherence without increasing the burden on clinicians.
Story Health has already demonstrated impressive results. Health systems utilizing their platform reported a 30-day heart failure all-cause readmission rate of just 6.9%, substantially lower than the national average of 18.1%. Furthermore, these systems observed over 60% reductions in hospitalizations and a notable improvement in daily patient engagement and medication adherence.
