U-Mamba2: Advancing Dental Anatomy Segmentation with State Space Models in CBCT Imaging

TLDR: U-Mamba2 is a new neural network architecture designed for accurate and efficient multi-anatomy segmentation in Cone-Beam Computed Tomography (CBCT) dental images. It integrates the Mamba2 state space model into a U-Net architecture, combining the strengths of CNNs for local features and Mamba2 for global dependencies. The model incorporates interactive click prompts, self-supervised pre-training, and dental domain knowledge (label smoothing, weighted loss for tiny structures, left-right mirroring augmentation, and post-processing) to improve performance. U-Mamba2 achieved top 3 rankings in both tasks of the ToothFairy3 challenge, demonstrating its effectiveness and efficiency for clinical applications like diagnosis and surgical planning.

Dental imaging plays a crucial role in modern dentistry, providing detailed 3D views of the jaw and teeth. Cone-Beam Computed Tomography, or CBCT, is a widely used technique that offers comprehensive volumetric information, essential for accurate diagnosis, treatment planning, and surgical procedures. However, precisely identifying and outlining individual anatomical structures within these 3D scans, a process known as segmentation, is a time-consuming and challenging task when done manually by experts.

To address this challenge, researchers have been developing automated solutions using neural networks. Traditionally, these networks fall into categories like Convolutional Neural Networks (CNNs), which are good at capturing local features, and Transformers, which excel at understanding global information. Hybrid architectures attempt to combine the strengths of both. While Transformers offer improved global feature capabilities, their computational demands, especially with high-resolution 3D data common in healthcare, can be a significant limitation.

A promising alternative has emerged in the form of structured state space sequence models, particularly the Mamba model. These models are known for their efficiency, scaling linearly with input size, and their ability to selectively capture relevant input features. Building on this, the Mamba2 model further refines this approach, dramatically improving speed without sacrificing performance.

Introducing U-Mamba2 for Dental Segmentation

A new neural network architecture, U-Mamba2, has been developed to enhance multi-anatomy CBCT segmentation. This innovative model integrates the Mamba2 state space models into the well-established U-Net architecture, creating a hybrid CNN-SSD (Structured State-Space Duality) framework. This combination allows U-Mamba2 to efficiently extract local spatial features using CNNs while simultaneously capturing global, long-range dependencies with Mamba2. The result is a system that enforces stronger structural constraints, leading to higher efficiency without compromising accuracy.

U-Mamba2 was specifically designed for the ToothFairy3 challenge, which involves segmenting a wide range of dental anatomies, including jaws, sinuses, 32 teeth, and more recently, pulps, incisive nerves, and the lingual foramen. The model also supports interactive segmentation, allowing users to provide click prompts to refine the segmentation of structures like the inferior alveolar nerves.

Key Innovations and Performance

The effectiveness of U-Mamba2 stems from several integrated innovations:

  • Mamba2 Integration: By placing the U-Mamba2 block, which contains the SSD-based Mamba2, in the bottleneck stage of the U-Net, the model efficiently processes image features as long sequences to capture global contexts.

  • Interactive Click Prompts: An optional branch with cross-attention blocks allows the model to incorporate user-provided clicks, enabling human-in-the-loop refinement and improved accuracy, particularly for challenging structures.

  • Self-Supervised Pre-training: U-Mamba2 is pre-trained using a disruptive autoencoder (DAE) framework on a large dataset of unlabeled CBCT scans. This process helps the model learn meaningful feature representations, which significantly boosts performance on downstream segmentation tasks, especially when labeled data is limited.

  • Dental Domain Knowledge: The model incorporates specific dental insights:

    • Label Smoothing: For anatomies with similar shapes or close structural relationships (e.g., left-right tooth counterparts, nerves), label smoothing helps the model recognize these similarities.

    • Weighted Loss for Tiny Structures: Smaller, sensitive structures like the incisive nerves and lingual foramen are given a higher weight in the loss function to ensure their accurate segmentation is not overshadowed by larger anatomies.

    • Left-Right Mirroring Augmentation: By carefully swapping class labels during data augmentation and inference, the model can exploit the anatomical symmetry of the orofacial region, significantly increasing its generalization capabilities.

    • Post-processing: A final step removes small, likely false-positive predictions based on anatomical priors, ensuring that segmented voxels belonging to the same anatomy are connected and coherent.

Extensive experiments, including independent tests, have demonstrated U-Mamba2’s superior performance. It secured top 3 places in both tasks of the ToothFairy3 challenge. For Task 1, it achieved a mean Dice score of 0.792 and an HD95 of 93.19. For Task 2, it achieved a mean Dice score of 0.852 and an HD95 of 7.39. The model also showed competitive inference times, demonstrating its efficiency in real-world applications.

The development of U-Mamba2 marks a significant step forward in automated dental anatomy segmentation, offering a robust and efficient tool that could greatly improve the accuracy and speed of dental care. For more technical details, the research paper can be found here.
