ReclAIm: An Autonomous Framework for Sustaining Medical Imaging AI Performance

TLDR: ReclAIm is a novel multi-agent framework designed to autonomously monitor, evaluate, and fine-tune medical image classification models. Built on a large language model core, it allows users to manage AI performance through natural language, eliminating the need for programming expertise. The system successfully detects performance degradation in models across MRI, CT, and X-ray datasets and applies state-of-the-art fine-tuning procedures to restore performance, ensuring long-term reliability in clinical practice.

In the rapidly evolving landscape of healthcare, Artificial Intelligence (AI) models are becoming indispensable, particularly in medical imaging. However, ensuring these AI systems maintain their accuracy and reliability over time is a significant challenge. Factors like shifts in data distribution, changes in imaging equipment, or modifications in clinical protocols can cause AI model performance to degrade. Addressing this critical need, a new multi-agent framework called ReclAIm has been developed to autonomously monitor, evaluate, and fine-tune medical image classification models.

ReclAIm stands out because it operates entirely through natural language interaction, meaning users don’t need programming expertise to manage complex AI maintenance tasks. This user-friendly approach makes advanced AI model management accessible to a broader range of scientific and clinical personnel.

How ReclAIm Works: A Multi-Agent System

At its core, ReclAIm is built on a large language model (LLM) that acts as the central reasoning engine. This LLM orchestrates a team of specialized agents, each equipped with specific Python tools to perform distinct functions:

  • The Image Classification Agent: This agent is responsible for training and deploying various deep learning models, such as ResNet, InceptionV3, VGG16, and EfficientNet. It handles everything from data augmentation to managing class imbalances, allowing users to train custom models with their own data.

  • The Performance Comparison Agent: This agent evaluates the models’ performance by comparing inference outputs against test results. It assesses global metrics like accuracy, precision, recall, and F1-score, as well as per-class performance, to detect any degradation when models are applied to new data. If a significant decline is found, it recommends corrective actions, including fine-tuning.

  • The Fine-Tuning Agent: When performance degradation is detected, this agent steps in to adapt and optimize the existing models. It employs various strategies, such as full or partial layer retraining, and integrates techniques to prevent ‘catastrophic forgetting’—a common issue where retraining for new data causes the model to forget previously learned information.

The system operates in a continuous cycle of ‘thinking-acting-observing.’ A user provides a natural language prompt to the master agent, which then analyzes the task, devises a strategy, and invokes the appropriate task-specific agents. The results of these actions are stored in memory, allowing the system to learn and adapt, either returning a final result or initiating a new cycle if the task is incomplete or errors occur. The LLM used for ReclAIm’s core reasoning is ChatGPT-4.1.

Real-World Application and Results

To demonstrate its capabilities, ReclAIm was tested across three diverse medical imaging datasets: a brain tumor MRI dataset, a chest CT dataset for COVID-19 diagnosis, and a chest X-ray dataset for pneumonia detection. The system successfully developed and evaluated 12 classification models (EfficientNet, InceptionV3, ResNet50, and VGG16 for each dataset).

A key part of the evaluation involved simulating performance degradation. ReclAIm was prompted to classify unseen inference datasets and then compare these results with baseline performance. When a decline greater than 5% in at least one metric was observed, the fine-tuning agent was activated. For instance, in the brain tumor MRI dataset, InceptionV3 showed a significant recall drop of 41.1% for one class. After ReclAIm’s autonomous fine-tuning, this performance gap was substantially reduced, with post-fine-tuning deltas confined to small magnitudes, often within ±1.5% of the initial model results.

The framework successfully readjusted performance metrics for five out of twelve models that showed degradation across MRI, CT, and X-ray modalities. This demonstrates ReclAIm’s ability to not only detect issues but also to implement state-of-the-art corrective procedures autonomously.

Implications for Clinical Practice and Regulation

The development of ReclAIm is particularly timely given the increasing adoption of AI in medicine and the emphasis on continuous monitoring by regulatory bodies like the EU AI Act and the FDA. The FDA’s guidance for Predetermined Change Control Plans (PCCP) highlights the need for adaptive retraining of AI algorithms, and ReclAIm provides an automated solution that aligns with these principles.

While current research often focuses on text-based AI agents in medicine, ReclAIm extends the application of agentic systems to more interactive and autonomous settings, managing end-to-end AI workflows for medical imaging. This capability is crucial for ensuring the long-term safety, compliance, and sustained clinical performance of AI models.

ReclAIm represents a significant step towards automated, continuous maintenance of medical imaging AI models, making them more reliable and adaptable for both research and clinical environments.
