Pic2Diagnosis: AI for Direct Heart Disease Detection from ECG Prints

TLDR: Pic2Diagnosis is a novel AI method that directly diagnoses cardiovascular diseases from printed or scanned ECG images, eliminating the need for digitization. It uses a two-step curriculum learning framework and an ensemble of three models to achieve high accuracy (AUC 0.9534, F1 0.7801) and robustness against real-world image artifacts. This approach simplifies the diagnostic process, enabling rapid and accurate CVD diagnosis, particularly beneficial in resource-limited settings.

Cardiovascular diseases (CVDs) remain a leading cause of death globally, underscoring the critical need for accessible and accurate diagnostic tools. The electrocardiogram (ECG) is a fundamental instrument in diagnosing heart conditions. However, a significant challenge in real-world clinical settings, especially in areas with limited resources, is the reliance on printed or scanned ECG outputs rather than high-quality digitized signals. These physical records are often prone to artifacts like blurring, rotations, and contrast variations, which can hinder the accuracy of traditional machine learning approaches.

Addressing this challenge, a new method called Pic2Diagnosis has been developed for the direct diagnosis of cardiovascular diseases from these printed ECG images, completely bypassing the need for a digitization step. This innovative approach aims to simplify the diagnostic process while maintaining high accuracy, thereby expanding access to reliable cardiovascular care in diverse clinical environments.

How Pic2Diagnosis Works

The Pic2Diagnosis method employs a sophisticated two-step curriculum learning framework. Initially, a classification model is pre-trained using segmentation masks, which is a relatively simpler task. The knowledge gained from this initial training is then transferred to a new model. This new model is fine-tuned to operate directly on grayscale, inverted ECG images, making them resemble the segmentation masks and improving the learning process.

Before the images are fed into the models, they undergo several preprocessing steps to enhance their quality and prepare them for analysis. These steps include Contrast Limited Adaptive Histogram Equalization (CLAHE) for contrast enhancement, removal of the background to isolate the ECG paper, perspective transformation to correct for rotations, grayscale conversion, and color inversion. These steps ensure that the model receives a consistent and optimized input, even from imperfect real-world images.

To further boost robustness and overall performance, Pic2Diagnosis utilizes an ensemble learning approach. This means that three versions of the final model, each trained with slightly different settings, are used. Their individual predictions are then averaged, leveraging complementary insights from each model to provide a more reliable and accurate diagnosis. This ensemble strategy has proven to significantly improve the model’s ability to generalize to unseen data and mitigate weaknesses found in individual models.

Performance and Impact

The effectiveness of Pic2Diagnosis was rigorously tested on the BHF ECG Challenge dataset. The ensemble model achieved an impressive Area Under the Curve (AUC) of 0.9534 and an F1 score of 0.7801 on the validation set, outperforming individual models. On the held-out test set, the ensemble model maintained strong performance with an AUROC score of 0.93786 on the private portion and 0.94715 on the public portion. These results highlight the method’s capability to handle real-world artifacts and deliver accurate diagnoses.

This automated procedure offers a reliable solution for CVD diagnosis, particularly in resource-limited settings where printed or scanned ECG images are common. By simplifying the diagnostic workflow and providing rapid, accurate results, Pic2Diagnosis is critical for timely intervention in CVD cases that often demand urgent care.

Future work aims to enhance the explainability of the model’s decisions, providing clinicians with greater insight into the AI’s reasoning. This will foster trust and facilitate the integration of such advanced diagnostic tools into everyday clinical workflows, especially for critical conditions like myocardial infarction (MI) and atrial fibrillation (AF).
