Med-CTX: A Multimodal AI Framework for Explainable Breast Cancer Ultrasound Analysis

TLDR: Med-CTX is a novel AI framework that combines breast ultrasound images with clinical radiology reports to provide highly accurate and explainable breast cancer lesion segmentation. Utilizing a dual-branch transformer architecture and integrating BI-RADS standards, it generates precise segmentation masks, uncertainty maps, and clinically grounded diagnostic explanations. This approach significantly outperforms existing methods in accuracy and interpretability, aiming to build trust and improve decision-making in medical diagnostics.

Artificial intelligence holds immense promise for medical imaging, but its widespread adoption in clinical settings has been hampered by a significant trust gap. Clinicians need more than just accurate predictions; they require clear explanations, reliable confidence estimates, and alignment with established medical standards. Traditional AI systems often act as ‘black boxes,’ providing results without revealing how they arrived at their conclusions, which is a major concern in critical medical environments.

This challenge is particularly evident in breast cancer diagnostics, where standardized reporting systems like the Breast Imaging Reporting and Data System (BI-RADS) are crucial. Despite their importance, BI-RADS descriptors are rarely integrated into deep learning models, leading to issues such as diagnostic variability among radiologists, potential delays in cancer diagnoses due to false negatives, and an increase in unnecessary biopsies from false positives. Radiologists often distrust AI predictions that lack BI-RADS concordance, limiting the clinical utility of even highly accurate automated systems.

Addressing these critical gaps, researchers have introduced Med-CTX, a groundbreaking, fully transformer-based multimodal framework designed for explainable breast cancer ultrasound segmentation. Med-CTX stands out by integrating clinical radiology reports directly into its processing, significantly boosting both its performance and its ability to provide understandable explanations. This framework generates segmentation masks, uncertainty maps, and diagnostic rationales simultaneously, enhancing confidence and transparency in computer-assisted diagnosis.

How Med-CTX Works

Med-CTX employs a sophisticated architecture that processes both grayscale ultrasound images and clinical text. Its core design features a dual-branch visual encoder that combines Vision Transformer (ViT) and Swin Transformer models. ViT excels at capturing global context within an image, while Swin Transformer is adept at preserving fine-grained local details, such as lesion boundaries. This combination ensures a comprehensive understanding of the visual data.

A key innovation is the integration of clinical language. Structured clinical text, particularly BI-RADS semantics, is encoded using BioClinicalBERT, a language model specialized for medical terminology. This textual information is then combined with visual features through a process called cross-modal attention. This allows the model to provide explanations that are clinically grounded and directly related to the input data.

The framework operates in three main stages: encoding, fusion, and decoding. First, visual and textual inputs are independently processed into aligned feature spaces. Second, these multimodal features are fused using an uncertainty-modulated cross-attention mechanism, which helps the model suppress uncertain regions and enhance robustness. Finally, a shared transformer decoder produces the segmentation masks, pixel-wise uncertainty maps, and clinical predictions, including pathology, BI-RADS category, and confidence scores.

Med-CTX also incorporates a dual-pathway explanation generation system. It uses a neural language generator (GRU decoder) to produce free-text reports and combines this with structured clinical reasoning based on BI-RADS rules to generate templated phrases. The result is a coherent, clinically aligned narrative that explains the diagnostic findings.

Performance and Impact

Evaluated on the BUS-BRA dataset, a comprehensive collection of breast ultrasound images with BI-RADS annotations, Med-CTX achieved remarkable results. It recorded a Dice score of 98.79% and an Intersection over Union (IoU) of 95.18% for segmentation, significantly outperforming existing baselines like U-Net, Swin, and ViT. This superior performance highlights the effectiveness of its dual-branch encoder and adaptive fusion gate.

Crucially, Med-CTX also demonstrated excellent textual explanation quality. It achieved a CIDEr score of 0.58, a 19% improvement over previous models like MedCLIP, indicating its ability to generate coherent and clinically relevant rationales. The model’s confidence calibration was also dramatically improved, ensuring that its reported confidence accurately reflects its true reliability, which is vital for clinical trust.

Ablation studies confirmed the critical role of clinical text, showing a significant drop in performance when it was removed. This underscores Med-CTX’s unique strength in leveraging multimodal data for enhanced accuracy and interpretability.

By integrating BI-RADS semantics, clinical text, and uncertainty-aware reasoning, Med-CTX effectively bridges the trust gap in medical AI. Unlike opaque ‘black-box’ models, it offers explainable decisions, actionable uncertainty estimates, and alignment with domain standards. This modular design also paves the way for future extensions, such as integration with 3D ultrasound and real-world radiology reports, promising a new era of trustworthy and transparent AI-assisted diagnostics.
