Bridging Medical Ontologies for Enhanced Healthcare AI

TLDR: LINKO is a new framework that improves how medical concepts are represented in electronic health records (EHRs). It integrates multiple medical ontologies (like diseases, drugs, procedures) by allowing knowledge to flow both within each ontology (vertical propagation) and across different ontologies (horizontal propagation). It also uses large language models (LLMs) to provide initial, context-rich embeddings for medical concepts. This dual-axis approach and LLM augmentation lead to more robust and accurate predictions in healthcare tasks, especially for rare conditions and with limited data.

In the rapidly evolving landscape of modern healthcare, electronic health records (EHRs) have become a cornerstone for developing advanced machine learning models. These models hold immense potential for predicting various clinical outcomes, from disease diagnoses to treatment recommendations. However, the inherent complexity of medical concepts within EHRs—characterized by their diversity, sparsity, and temporal variability—presents a significant challenge, especially when dealing with infrequent codes like those for rare diseases.

A promising avenue to address this challenge involves integrating domain knowledge, particularly the structured medical knowledge provided by ontology graphs. Ontologies are essentially hierarchical systems that organize clinically related concepts, mapping external knowledge to medical codes through structured relationships. While existing research has explored incorporating domain knowledge, it often focuses on single ontology systems or treats multiple systems (like diseases, drugs, and procedures) in isolation. This approach limits concept representation learning to relationships within a single ontology, overlooking crucial connections that span across different medical domains.

Introducing LINKO: A Unified Framework for Medical Concept Representation

To overcome these limitations, researchers have proposed LINKO (LLM-augmented INtegrative Knowledge Propagation over Ontology Graphs), a novel framework designed to enhance medical concept representation learning. LINKO stands out by simultaneously leveraging multiple ontology graphs and enabling a unique dual-axis knowledge propagation system—both within and across heterogeneous ontology systems.

The framework operates in three key steps:

1. Meta-KG Construction and LLM Initialization: LINKO begins by building a comprehensive ‘Meta-Knowledge Graph’ (Meta-KG) that integrates various ontology graphs across every level of their hierarchy. Crucially, the initial embeddings (numerical representations) for each medical concept in this Meta-KG are generated using large language models (LLMs). This is achieved through a carefully crafted ‘graph-retrieval-augmented prompt’ that includes concept descriptions, ontology relations, and task-specific details. This step injects rich clinical knowledge from LLMs into the concept embeddings, providing a strong foundation for subsequent learning while minimizing the risk of factual inconsistencies often associated with free-text LLM outputs.

2. Horizontal Message Passing (HMP): This step focuses on linking medical concepts within and across different ontologies at all hierarchical levels. It leverages observational co-occurrence patterns found in EHR visits to establish connections between frequently co-occurring concepts. For instance, a disease and a drug often prescribed together would be linked. This horizontal propagation helps fuse diverse ontologies (diagnosis, drug, procedure) and captures heterogeneous interactions, utilizing information at both fine-grained and coarse-grained levels of granularity.

3. Vertical Message Passing (VMP): Complementing HMP, VMP facilitates information flow across the hierarchical levels within each individual ontology. This two-round process starts with ‘bottom-up propagation’ (Hierarchical Graph Information Propagation or HGIP), where information flows from child concepts to their more general parent concepts. This is followed by ‘top-down propagation’ (GRAM), which passes updated information back to the children, ultimately refining the representations for the most specific medical codes (leaf nodes).

Enhanced Performance and Robustness

Extensive experiments conducted on two widely used EHR datasets, MIMIC-III and MIMIC-IV, demonstrate LINKO’s superior performance compared to state-of-the-art baselines. When integrated as a plug-in encoder, LINKO consistently boosts the predictive accuracy of various existing EHR models, validating its effectiveness in enhancing concept representation learning. The framework also shows enhanced robustness in scenarios with limited data availability and significantly improves the prediction of rare diseases, a common challenge in healthcare AI.

The ablation studies further highlight the critical role of each component within LINKO. Removing either horizontal or vertical message passing, or the LLM-augmented initialization, leads to a noticeable drop in performance, underscoring the synergistic benefits of these integrated features. The research also explored different prompting strategies for LLMs, finding that including detailed concept information, parent codes, and task names yields the most effective embeddings.

By providing a unified learning structure that integrates diverse medical ontologies and leverages the power of large language models, LINKO offers a significant advancement in how medical concepts are represented and understood in healthcare AI. This approach promises richer, more robust, and more accurate predictive models, ultimately contributing to better patient care. You can find the full research paper here.
