TLDR: FundusExpert is a new AI model for ophthalmology that improves eye disease diagnosis by integrating positioning-diagnosis reasoning. It uses a novel dataset, FundusGen, built with Fundus-Engine, which automates image analysis and aligns AI reasoning with clinical thought processes. This approach leads to significantly higher accuracy in diagnosis and report generation compared to other models, demonstrating the importance of data quality and cognitive alignment in medical AI.
Artificial intelligence is making significant strides in medicine, and a new development in ophthalmology promises to enhance the accuracy and interpretability of eye disease diagnosis. Researchers have introduced FundusExpert, a specialized AI model designed to improve how multimodal large language models (MLLMs) understand and interpret fundus images, which are crucial for diagnosing various eye conditions.
Traditional MLLMs often struggle in specialized medical fields like ophthalmology due to fragmented data annotations and inconsistencies in how they mimic clinical reasoning. This can lead to less precise diagnoses and difficulty in understanding *why* the AI made a certain decision. FundusExpert aims to bridge this gap by integrating “positioning-diagnosis” reasoning, meaning it can understand both the location of abnormalities and their diagnostic implications.
A key component of this breakthrough is FundusGen, a new dataset created using an intelligent system called Fundus-Engine. Fundus-Engine automates the localization of features within fundus images and uses AI to expand semantic information. This system integrates global disease classification, local object detection, and fine-grained feature analysis from a single image. Crucially, it constructs a “clinical cognitive chain,” guiding the AI model to generate reasoning paths that are similar to how a human clinician would think.
This cognitive chain helps the model move from observing specific regions to analyzing features and then to making a diagnostic reasoning. For instance, it can first identify an abnormal region, then analyze its characteristics, and finally suggest a diagnosis. It also allows for multi-turn dialogues, where the AI can refine its diagnosis by verifying fine-grained features, much like a doctor would ask follow-up questions.
FundusExpert, which was fine-tuned using instruction data from FundusGen, has shown impressive results. In ophthalmic question-answering tasks, it significantly outperformed existing models, including larger ones. For example, it surpassed the average accuracy of the 40B MedRegA model by 26.6%. It also excelled in generating medical reports without prior examples, achieving a clinical consistency of 77.0%, which is much higher than GPT-4o’s 47.6%.
The research also revealed an important “scaling law” for medical multimodal data, indicating that the quality of data, especially data aligned with clinical cognitive logic, is more important than just the sheer volume of data. This means that carefully curated datasets like FundusGen can lead to more efficient and effective AI models.
